• Intake, scheduling, customer-relationship-management, communication, and follow-up providers, including PT Marketing Machine;
• Billing, coding, Medicare, eligibility, and claims-support services;
• Payment processors and financial institutions;
• Email, telephone, text-messaging, and electronic-fax providers;
• Information-technology, cybersecurity, cloud-storage, backup, and data-hosting providers;
• Accounting, legal, consulting, and compliance professionals;
• Analytics and website-performance providers; and
• Document-storage, mailing, and records-management providers.

These providers may access information only as reasonably necessary to perform services for us or as otherwise permitted by law. When a provider creates, receives, maintains, or transmits protected health information on our behalf, we require a Business Associate Agreement when HIPAA requires one.

Performance Edge PT does not sell personal information for money.

We do not share mobile telephone numbers, text-message opt-in information, or text-message consent with third parties or affiliates for their own marketing or promotional purposes. We may provide this information to service providers that support delivery of our communications, such as telecommunications carriers and messaging-platform providers, but they may use it only to provide those services to us.

COOKIES, ANALYTICS, AND TRACKING TECHNOLOGIES

EMAIL, TEXT MESSAGES, AND TELEPHONE COMMUNICATIONS

If you provide your contact information, we may communicate with you about inquiries, scheduling, intake forms, appointments, waitlist status, services, billing, and other matters related to your relationship with Performance Edge PT.

Where required, we will obtain separate consent before sending automated or promotional text messages. Consent to receive promotional text messages is not a condition of receiving treatment or purchasing services.

Message frequency may vary. Message and data rates may apply. You may reply STOP to a text message to opt out of future nonessential texts and HELP for assistance. Opting out of promotional messages will not prevent necessary individualized communications that are otherwise permitted by law, although we will honor applicable communication preferences and opt-out requirements.

Email and ordinary text messaging may not always be fully secure. Consider the sensitivity of information before sending detailed health or financial information through an unencrypted message.

PAYMENT INFORMATION

Payments may be processed by third-party payment processors. Performance Edge PT may receive transaction information such as the amount paid, payment status, card type, and limited account details.

We generally do not directly retain complete payment-card numbers when payments are processed through a third-party payment processor. Payment processors handle payment information according to their own privacy and security practices.

DATA SECURITY

We use reasonable administrative, technical, and physical safeguards designed to protect personal information. These measures may include access controls, passwords, secure systems, vendor management, staff training, backups, and other safeguards appropriate to the nature of the information.

CHILDREN’S PRIVACY

Our general website is not directed to children under 13, and we do not knowingly collect personal information directly from children under 13 through the general website without appropriate parental involvement.

Performance Edge PT may provide services to minors when appropriate. Information about a minor patient should be submitted by a parent, legal guardian, or other legally authorized person, except where applicable law permits the minor to act independently.

DO NOT TRACK AND BROWSER SIGNALS

Some browsers offer “Do Not Track” or similar preference signals. Because there is not a uniform standard for all such signals, our website may not respond to every signal.

Where applicable law requires recognition of a particular legally valid opt-out preference signal, we will process it as required.
